At CNHPC we have established a complete pain center. Whether you are referred to me by your personal physician or here on a self-referral basis, we will thoroughly discuss all details of your condition beginning with a complete neurological work-up. You will be tested on alertness, memory, speech, vision, strength, coordination, reflexes, and sensation. After a diagnosis is determined, additional tests may be needed to confirm the diagnosis and to define treatment. Common neurological tests include:

• Evoked potentials (VEP, BEP, SSEP)
• Electroencephalogram (EEG)
• Carotid ultrasound
• Transcranial dopplers
• Patent foramen ovale (PFO)
• Electromyogram (EMG)
• Nerve conduction velocities (NCV)
• MRI/MRA

• Spect scan
• Neuropsychological testing

What to Bring to Your Appointment

• Your insurance card
• Physician referral forms if required by insurance
• A list of current prescription or over-the-counter medications you are taking, including dose and frequency
• Pertinent information about your medical and surgical history.
• Any recent x-rays or appropriate records you may have.

We request payment at the time of your appointment for services that are not covered by insurance. Your insurance company may also require you to pay a co-payment at the time of your appointment. When necessary, our staff will work closely with patients who require a payment plan.
